Medical Training Google Ads Case Study: 100% Qualified US Leads
A US-based training center offers in-person courses in joint injection techniques for physicians and chiropractors, drawing specialists from across the country.
When the client reached out to us, Google Ads was already running. However, the client was unsatisfied with both the quantity and quality of leads: the budget was being spent, but there was no steady flow of qualified inquiries.
Our goal was to increase the number of Google Ads leads and attract professionals who were genuinely interested in the training.
Goals and Objectives
Workflow
When the client transferred to us from their previous agency, the ad account showed over 20 conversions per week at a cost of roughly $20 each. Based on the numbers, it might have seemed like the ads were performing well. However, the client saw a different reality: the majority of inquiries were unqualified. Therefore, our first step was to audit the active ad campaigns and verify where the inquiries were coming from and whether Google was tracking them correctly.
Key Challenges
During the audit, we uncovered several issues that prevented an accurate evaluation of ad performance and hindered generating more qualified leads:
- A high volume of non-targeted search queries. Ads were shown to people searching for other services or general information. These clicks consumed the ad budget but rarely converted into relevant clients;
- Lead tracking error. The ad account had an action configured that Google counted as a lead, even if the user never submitted the form. As a result, the number of actual inquiries in the ad account was inflated;
- Complex target geography. The training is held in person, but potential attendees are located all across the US. It was not enough for a person to simply be interested in the course—they had to be willing to travel from another city or state.
Due to these factors, relying solely on the volume and cost per conversion in Google Ads was impossible. We first needed to clean up the analytics and lead quality.
How We Fixed the Situation
Setting Up Accurate Analytics
Our first step was fixing the lead tracking. We configured tracking so that only real user inquiries were reflected in the stats, ensuring actions without an actual form submission were no longer counted as leads. As a result, we obtained accurate data: it became clear how many inquiries the ads were truly bringing in and how much a real lead actually cost. With this baseline, we could properly evaluate the ad campaigns and decide which ones to scale and which ones to pause or refine.
Campaign Cleanup
Next, we tackled the search queries. During the first two weeks, we conducted a thorough cleanup of the ad campaigns, excluding a large volume of queries that were attracting the wrong audience. Following the initial cleanup, we continued to regularly review lead quality and filter out irrelevant search queries. As a result, less ad budget was wasted on users for whom the client’s training was not a good fit.
Ad Optimization
After fixing the analytics and cleaning up the traffic, we continued optimizing the campaigns based on real leads. The primary focus shifted from a high volume of cheap conversions to the quality of incoming inquiries. This was especially vital for an in-person training project, as the client did not just need anyone who filled out a form, but physicians or chiropractors who were genuinely interested in the course and ready to travel for it.
Results
Following optimization, the ads reached a stable performance level: After correcting the setup, the total number of leads decreased and the cost per lead increased. However, the client began receiving significantly higher-quality inquiries—from specialists who actually purchased the training.
Summary
This project highlighted why a low cost per lead does not guarantee strong ad performance. If Google shows plenty of cheap conversions, but some are tracked incorrectly and the rest are not a fit for the business, relying on those metrics is a mistake.
